Medication Forms

If your child takes medication on a regular basis, this should be indicated on the medical forms.  To have a medication administered by staff at school, please fill out the appropriate form below: the Long-Term Medication Form for medications that are given for a month or more; or the Short-Term Medication form for medicines given for less than two weeks. All prescribed medicines must come in their original prescription bottle with the child’s name and dose on the bottle.

Uniform Policy

The primary and elementary classroom uniform consists of a solid light blue or solid white top and navy-blue slacks, skirt, shorts or jumper. No stripes, no polka dots, no other designs. No blue jeans. The best shoes for the children are tennis shoes, which allow them to run and climb outside and walk gracefully in the classroom. If snow or rain boots are worn to school, please also bring tennis shoes for the classroom. Children are allowed to wear socks in the classroom, but bare feet are not permitted.

The Erdkinder (7th-12th grade) uniform consists of a collared, solid white, maroon, or blue shirt with khaki or navy skirts or slacks.  In cold weather months, a solid white, blue or maroon sweater or sweatshirt may be worn with the uniform shirt collar showing. The sweater or sweatshirt must be without a hood and blank (no writing, logos or advertising). Tennis shoes are also worn.
